Reception Information Sheet All of us at RiverCity Entertainment, want to make your wedding reception a complete success. You can help us by completing this form. With this information, your RiverCity Entertainment DJ will be able to conduct your custom-tailored reception in a smooth and organized manner. We would like to have this form filled and email to us (by clicking submit when finished), no later that 21 days before your scheduled reception. NOTE: This form is for information only and is not part of the contract or reservation. Once we receive this form we will then put together the event itinerary from the information gathered from this form and the consultation meetings. We look forward in making you reception a memorable one, we will see you on the dance floor.
